services. Your Case Manager is dedicated to keeping you at home and will
make sure that you receive all of the clinical services and supplies

Whether you are recovering
from a hospital stay, newly diagnosed with medical condition, or living
with a chronic disease, our nurses, therapists and your physician, will
work together to make sure you are able to remain healthy and safe at
home.
A Team Approach To Caring:
Your Physician is
responsible for oversight of your care plan and together with your
Nurse, he/she will make sure that you receive the care necessary to
keep you healthy and safe at home. Your physician will receive regular
reports form the Nurses, Therapist or Medical Social Worker. He/she will
give members of your care team orders for medications, treatments or
visit frequencies to ensure that you receive the care you need.
Your care team will
encourage you to keep you doctor appointments, but if you are not
feeling up to it, they will perform the necessary tests right at home
and give your doctor a full report. When you are able to see your
physician, your care team will make sure he/she has all of your current
health information.

Our Services:
We offer home health
care services with care being coordinated by professional case managers
between staff, physicians, clients, and family. These physician ordered
visits promote, maintain, and restore health to our patients all within
the comforts of their own home. A few of the services provided by home
health include health assessments, dietary counseling, wound care,
muscle strengthening.
Our team of caregivers includes healthcare professionals who can provide the following services:
Skilled nursing:
Nurses work with the patient, family and physician to develop a
plan of care designed to restore the patient to their maximum
level of independence at home. Skilled nursing services include
physical assessment, health teaching, nursing procedures, and
monitoring patients with complicated medical needs.
Physical
Therapy:
Physical Therapists evaluate the patient and together with the
physician develop an exercise program that will ensure that
patient’s remain active and safe in their homes. Physical
Therapy services include balance and coordination exercises,
gait training, teaching the use of assistive devices and home
safety.
Occupational Therapy:
Occupational Therapists work with patients who have lost all or
some of their upper body function. Occupational Therapists work
with patients to help them regain independence with activities
such as grooming, bathing, dressing and other day-to-day
activities. Occupational Therapists also teach patients safe
use of assistive devices.
Speech Pathology:
Speech Pathologists provide assessment and evaluation for
patients who have experienced neurological and cognitive
disorders such as a stroke. Speech Pathologists work on both
cognitive (memory, learning, communication) and functional
(swallowing and speech) impairments. Their goal is for the
patient to be safe at home and to regain the ability to
communicate with loved ones.
Home Health Aides:
Certified Home health aides assist patients with grooming,
bathing, toileting and other personal cares as assigned by the
nurse. These services not only help the patient feel better but
also contribute to the patient's overall health and well-being.
Medical Social Work:
Medical Social Workers work with the patient and family to
provide information about community resources, financial
planning, and various support groups. Medical Social Workers
also provide limited counseling for patients and families in
need of support services.
Patient& Family Education:
Patient and family education is an important
part of home health care. The involvement of the patient and
family members in the treatment plan often improves the
effectiveness of the plan. |
