Home Physicians Physician Information

Physician Requirements For Home Health Services:
Staff members identify not only the clinical needs of the patient, but also the social and spiritual needs to assure a comprehensive plan of care and a positive patient care experience. This approach will enrich the patient while facilitating their recovery and ensuring continuity of care.

The following provides information about physician involvement in using home care covered by Medicare, and securing reimbursement for physician time.

Care Plan Oversight (CPO):
Physician involvement in ongoing oversight of Medicare-covered home nursing services and reimbursement of physicians for time involved in this activity.


Physician Orders for Home Health Services:
Physicians are required to give orders for patient care in Field 21 of the Home Health Plan of Care (CMS 485). The home health agency will fill in this section of the form. However, you are required to read it carefully, strike out those items that are not appropriate, and add items that are appropriate for the proper care of your patient.


The physician orders must:

  • List frequency and duration of each discipline ordered; include frequency and duration for Physical Therapy, Speech-Language Pathology, or Occupational Therapy.

  • Denote the number of visits in days, weeks, or months.

  • Include all disciplines and services/treatments to be rendered, even if they are not billable to Medicare.

  • Have qualified and quantified PRN orders (standing orders). Open-ended PRN orders are not acceptable.

You must re-certify the plan of care every 62 days. Always be sure to read the CMS 485 carefully before signing it.


Medicare Eligibility:
Medicare pays for healthcare in a patient’s home only if all four of the following conditions are met:

  1. The patient needs intermittent skilled nursing care, physical therapy, speech therapy or continuing occupational therapy.

  2. The patient is homebound.

  3. The patient is under the care of a doctor who determines the need for home healthcare and subsequently sets up and periodically reviews a home health plan of care.

The home health agency serving the patient is approved for payment by the Medicare program.
